An application must include:

  • This completed application form.
  • Résumé (limit to two pages).
  • Statement of personal and professional reasons for wanting to participate (limit to 500 words).
  • Any other evidence of teaching success, academic capabilities, a well-rounded personality, and the ability to work with other people.
To be mailed separately and directly:
  • Certified undergraduate and graduate transcripts mailed by registrars from institutions at which you completed a significant amount of course work. If your name has changed since university experience, please indicate transcript name.
  • Letters of recommendation from your immediate supervisor or principal and from one other person competent to judge your character and academic skills. At least one of these letters must be from someone with whom you have worked within the past two years. Letters will be completely confidential.

If selected, and if I accept, I agree to register for the 2008 Monticello-Stratford Hall Seminar which carries six semester hours of graduate credit at the University of Virginia at a tuition cost to me of $650. I also understand that my application will not be returned.
