Transferring a Stock Certificate
Transferring a stock certificate that is in the donor's name requires one of the following actions:
- Sending the unendorsed certificate along with a signed stock
power form that transfers the stock to the "Thomas Jefferson
Foundation." Please note that no signature
guarantee is required on either document, but that the donor
must sign the stock power form exactly as the name appears on
the face of the stock certificate. The certificate and the stock
power form should be mailed separately,
by registered mail or overnight delivery.
- Sending the donor-endorsed certificate (endorsed on the reverse
side of the certificate) and a Letter of Authorization from
the donor that indicates the following: 1) the number and name
of the shares contributed, 2) the certificate number, and 3)
the intent of the donor to contribute the shares to the Thomas
Jefferson Foundation. Note that the endorsement on the reverse
side of the certificate requires a signature
guarantee (the Letter of Authorization does not require
a signature guarantee). Both the endorsement on the reverse
side of the certificate and the Letter of Authorization must
be signed exactly as the name appears on the face of the certificate.
Please ensure secure handling of the certificate once it has
been endorsed, as it becomes a bearer certificate once the reverse
side in endorsed.
- Sending the donor-endorsed certificate (endorsed on the reverse side), which has also been assigned on the reverse side to the "Thomas Jefferson Foundation." Note that this type of endorsement and assignment on the reverse side of the certificate requires a signature guarantee and the endorsement must be signed exactly as the name appears on the face of the certificate.

Signatures
guarantees for stock certificates can be obtained at eligible
guarantor institutions, such as banks, stockbrokers, savings and
loan associations, and credit unions with membership in an approved
signature guarantee medallion program.
